You can request a new health insurance card by logging on to www.SouthCarolinaBlues.com.  Register online and use My Insurance Manager  to order a new insurance card.

Participation in Medi-Call is mandatory.  The State Health Plan has established a list of services for which a subscriber and covered dependents are required to call medi-call for precertification.  You can find this list in your Insurance Benefits Guide on Page 18.  Remember, it is your responsibility to make sure Medi-Call is contacted when required.  If you do not call Medi-Call when required, you may be subjected to penalties.  You will be required to pay a $200 penalty for each hospital or skilled nursing facility admission.  In addition, the $2,000/$4,000 coinsurance maximum will not apply.  Medi-Call can be reached at 1-800-925-9724 or 803-699-3337.

If you do not call APS Healthcare for approval for each mental health or substance abuse admission, services will not be covered.  APS can be reached at 1-800-221-8699 for recertification.
